Rexdale Community Health Centre

The Rexdale Community Health Centre’s latest CHC satellite is the Rexdale Community Hub on Panorama Court in the northwest of the city of Toronto.

Rexdale CHC supports and advocates for the physical, economic, social and mental wellbeing of its diverse community through primary health care, community social support, health promotion, collaborations and partnership, community development and social action.

Rexdale CHC delivers integrated, coordinated clinical services through a team of physicians, nurse practitioners, nurses, dietitians, social workers, a physiotherapist and a chiropodist.

The project also includes a community teaching kitchen and associated program space.
